Order Form for MultiColor Standard Size Wooden Nickels (1.5" diameter)

Standard Size Wooden Nickels are 1.5 inches in diameter with a 1.25" print size. (1.2 SqIn)
Jumbo Size Wooden Nickels are 2.0 inches in diameter with a 1.75" print size. (2.4 SqIn)
Curved Typesetting around the top and bottom curve is ALWAYS Free

Consecutive
Numbering
Consecutive Numbering can be added to either side for only
$6.85 per HUNDRED ($15.00 minimum set-up fee)
(Leave any 2 consecutive lines empty for the numbering)

Numbers can now be printed in Red, Black, Blue, or Green,
even if the rest of the nickel is a different color
Each side can be a different color at no additional charge
If both sides are the same color, the 2 sides may not match
since each side will be printed on a different type of printing press

Normally consecutive numbering starts with "0001"
but you may select any number you wish
The number can start with a "1" (ex. 10001)
or just have leading zeros (ex. 00001)
